We strive to make our appointment booking process as simple and stress-free as possible. While we welcome inquiries at any time, all patients are required to obtain a physician referral for treatment.  

You can speak with your physician to request a referral for Infusion treatment at the Smart Choice Fusion Center.  Your physician can complete a treatment referral form through our Provider Portal.
